The Problem with Refraction
For over one hundred years doctors have provided eye examinations including refraction by using lenses and prisms to determine the refractive state and binocularity of the patient. Persons with myopia (nearsightedness), hyperopia (farsightedness) and astigmatism are given a refraction to correct the refractive state and blurred vision of the patient by using lenses and prisms. While the phoropter and the auto-refractor are effective instruments and still used today, they still have limitations.
The Limitations of Refraction
- The peripheral field is blocked by the phoropter. This interferes with binocularity in any other meridian other than primary gaze or looking straight ahead. Since binocular imbalances are often present outside of the primary gaze position. The phoropter cannot provide a means of evaluation of binocularity outside of primary gaze.
- The large phoropter blocks the peripheral vision producing an abnormal environment and restriction of side vision. This can affect fusion and binocular testing as well as produce over-prescription during the refraction.
- Testing distance is limited to optical infinity or within 50 cm.
- Depth perception tests are limited and difficult to measure quantitatively.
- Depth of field cannot be tested in a phoropter or auto-refractor.
- Examination of children is limited with the auto-refractor and the phoropter and provides limited accommodation for interest and attention. The child must remain in one position for an extended period of time and this limits quantifying measurements.
- The present means of refraction utilizes 19th and 20th century technology. In order to increase efficiency, clinicians perform multiple examinations at one time but this requires clinically trained technicians to work with each patient. The auto-refraction and the subjective portions of the examination require one-to-one contact with the patient. In the present health care environment, this increases overhead expenses
